How to Prepare for Your I-693 Medical Exam: Complete 2026 Checklist

Arriving at your immigration medical exam prepared makes the difference between a smooth 7-10 day process and weeks of delays.

BEFORE YOUR APPOINTMENT

Documents to Bring

Form I-693 (Printed, UNSIGNED)

Download the latest version directly from USCIS : https://www.uscis.gov/i-693

  • Print single-side
  • Fill in YOUR information in Section 1 only (name, date of birth, country of birth, A-number if known)
  • DO NOT SIGN the form
Important: Form I-693 must be signed in the civil surgeon's presence per USCIS requirements. If you've already signed it, you will need to redo the form.

Government-Issued Photo ID

Acceptable:

  • ✅ Valid passport (preferred)
  • ✅ Driver's license (current state)
  • ✅ State ID card
  • ✅ Permanent resident card (Green Card) for spouses or family members in process
  • ✅ Travel document (refugee/asylum)

NOT acceptable:

  • ❌ Expired IDs
  • ❌ Photo of ID on phone
  • ❌ Library cards, work badges, school IDs

Vaccination Records

This document can save you hundreds of dollars for vaccines. The date of vaccination, the type of vaccine, and provider information are needed for documentation

Where to find your records:

  • Childhood immunization records (often kept by parents or in old school files)
  • Pharmacy immunizations records
  • Previous doctor's offices (pediatricians, family physicians, college health centers)
  • Records from previous immigration medical exams (if applicable)

Don't have vaccination records? We can:

  • Order blood tests confirm immunity for some vaccines (MMR, Hep B, Varicella, Polio)
  • Re-vaccinate (safe and effective even if you're already immune)
  • Wait for records to arrive — but this delays your I-693

List of Current Medications

Include prescription medications (name, dose, frequency), over-the-counter medications you take regularly, supplements and herbal remedies, and any recent antibiotic use (last 3 months).

Medical Records for Chronic Conditions

Most chronic conditions must be documented but are not disqualifying. Bringing records makes it easier to document them properly. If you have any of the following, please bring the additional documentation listed:

  • Diabetes- recent HbA1C, treatment plan
  • Hypertension- medication regimen
  • Heart conditions- treatment history
  • Asthma- treatment history
  • Mental health- treatment history
  • Past cancer- treatment summary and current status
  • Prior TB diagnosis and treatment
  • Hepatitis B or C
  • List of previous surgeries

Payment

We accept cash, credit/debit cards, HSA/FSA cards (we provide receipts for reimbursement). We do NOT accept personal checks.

We do NOT bill insurance.

Full payment is due at time of first visit.

What You Do NOT Need

Insurance card — immigration medical exams are considered administrative exams, insurance does not cover them.

❌

Fasting before appointment — NO fasting required! Please eat and be hydrated! You will have blood drawn and may need to give a urine sample.

IMPORTANT

Mistakes to Avoid

Mistake 1: Signing Form I-693 Before Your Appointment

USCIS requires the form to be signed in the civil surgeon's presence. If you've already signed, we may need to redo the form.

Mistake 2: Opening the Sealed Envelope

DO NOT OPEN the sealed envelope. USCIS requires it to be sealed for submission. If accidentally opened, contact us immediately — re-sealing may be possible depending on circumstances.

Mistake 3: Bringing an Insurance Card

We don't bill insurance for immigration medical exams. Some patients waste appointment time trying to use insurance cards that don't apply here.

Mistake 4: Fast for Labs

Fasting is NOT required for any lab work, and is done the same day, in our office unless you request an order to be done elsewhere. If labs are done elsewhere, you are responsible for making an appointment with the lab and providing the results to our office. Lab results are mandatory for Form I-693 completion.

Mistake 5: Hiding Medical History

"I'll just say no to mental health questions" or "I won't mention my past drug use." This is immigration fraud and far worse than the original issue. Most conditions don't disqualify you anyway.
